Your account and care information.

This notice covers account registration, the health questionnaire, and ASQ chat.

What we save

We save your full name, email address, password hash, and declared care-team role. We also save the cared-for person's full name, date of birth, care-location ZIP, and a record that you accepted the registration statements. Your password itself is not stored.

Why we ask

The information creates your login and starting care-team record. The date of birth helps identify whom your record is about. A matching name and birth date do not link you to another person's account.

Health questionnaire

You may choose to save your current concern, care setting, reported health conditions, equipment, support, and preferred language. These answers help establish the situation for future care guidance. Questions may be left blank. We save who supplied each version, when it was saved, and the statement accepted with it. Updating an answer adds a new version; earlier versions remain part of the stored record and backups. Answers are recorded as reported, not as a verified diagnosis.

Who can access it

You can view the care information saved through your own account. Authorized site operators can access stored records for administration and support. Declaring a professional or caregiver role does not grant access to other accounts.

ASQ and AI information sharing

Before your first ASQ message, we ask you to accept sharing chat information with OpenAI, which prepares the AI replies. Messages, relevant previous exchanges, care ZIP, a broad age group, and your declared care-team role are sent. If you choose to include saved care information, your latest questionnaire answers are also sent. Your account email, saved full names, and exact birth date are not automatically included. Anything you type in a message is sent, so leave out private identifiers such as member numbers.

ASQ can search the web for relevant information. Search instructions use general needs and location, rather than personal identifiers. We request that the API not store the response as retrievable conversation state. This is not a promise of zero retention: OpenAI processes submitted content under its API data policies.

Completed exchanges, source links, selected follow-up choices, the responsible account and the questionnaire version used are saved in our MySQL database and backups. Earlier chat messages can be used in later replies; turning off the saved-information checkbox stops adding the latest questionnaire, but does not remove information already in the conversation. AI replies are suggestions, not verified care outcomes. Your chat is private to your account; it is not sent to providers or insurers.

Where it is kept

The account database is hosted on our IONOS server in the United States. Connections to this website use HTTPS. Records remain stored after you log out, and server backups retain copies.

Current limits

ASQ replies require an active AI connection. The chat shows when that connection is unavailable. ASQ can help explain options and next steps; it does not diagnose, prescribe, confirm insurance coverage, make bookings or order equipment. Email ownership, caregiver authority and professional credentials are not verified. Online account recovery, registration-detail corrections, and account deletion controls are not yet available. Keep your login in your password manager.

Enter another person's information only when you have permission. Registration does not itself establish legal authority to make care decisions.
